Mission Statement
The mission of  Duke Orthopaedic (Hand Surgery surgical microscope,Operation microscope, Operating microscope)hand,Upper Extremity,and Microvascular fellowship Training program is to provide the trainee with the knowledge, clinical and surgical skills in all areas of hand, upper extremity,and microvascular (hand Surgery surgical microscope,operation microscope, Operating microscope) necessary to ethically practice either academic or community practice at the highest possible level of competence by the completion of the training program.

Program Description
The Duke Orthopaedic (Hand Surgery surgical microscope,Operation microscope, Operating microscope) Hand, Upper Extremity, and Microvascular  Fellowship Training Program is one year in length, from August 1 through July 31, under the direction of David S. Ruch, M.D., Professor of Orthopaedic and Hand Surgery (Hand Surgery surgical microscope, Operation microscope, Operating microscope) microscope.This fellowship is ACGME accredited (Accreditation Council for Graduate Medical Education) program number 2633621022 for three (3) positions and participates in the National Resident Matching Program.

The purpose(Hand Surgery surgical microscope,Operation microscope, Operating microscope)of this fellowship is to offer the Fellow special knowledge and training in the management of acute hand trauma,reconstructive hand problems,congenital deformities, and all aspects of micro including replantation,reconstructive,free tissue transfers and microneuropathy(Hand Surgery surgical microscope, Operation microscope, Operating microscope), which will allow the trainee to become expertly competent in the practice of the same. This educational program should provide experience in the repair, resection, and reconstruction of defects of form and function of the hand; in the design, construction, and transfer of flaps and the transplantation of tissues, including micrology (Hand Surgery surgical microscope, Operation microscope, Operating microscope)of multiple tissues; in replantation of upper extremity parts; in  ancillary methods of treatment of tumors; in management of complex wounds; and in the use of alloplastic materials.

A weekly conference(1 hour)dedicated to hand surgery (Hand Surgery surgical microscope, Operation microscope, Operating microscope) covering all aspects of hand surgery includes reconstructive, trauma, tumor, congenital, micrology, and rehabilitation. Also a monthly journal club is included as part of the Monday conference schedule. Each rotation for the residents is 6 weeks in duration and for the fellows is 2 months in duration.Four residents(hand Surgery surgical microscope,operation microscope,operating microscope) and three fellows are each assigned to one or more faculty member (s) who has some or all of their practice in hand and upper extremity.Trauma call schedule is divided to specific hand call group, which includes a dedicated attending and hand fellow daily. Hand call includes replant (Hand Surgery surgical microscope, Operation microscope, Operating microscope)services for which our institution is a major referral center for the entire southeast United States. Each fellow is responsible for presenting papers for journal club and presenting at least one conference.

Adult reconstruction (Hand Surgery surgical microscope, Operation microscope, Operating microscope) includes experience in treatment of osteoarthritis (including post-traumatic) and rheumatoid arthritis for the upper extremity,Dupuytren’s contracture,nerve compression syndromes,scleroderma & vascular insufficiency,and tendon reconstructions. Trauma includes treatment of fractures and dislocations of the upper extremity, flexor and extensor tendon injurie(Hand Surgery surgical microscope,operation microscope,operating microscope),nerve injuries, compartment syndromes, and replantation services.

Congenital hand includes treatment of all birth related injuries such as congenital absence of the thumb(Hand Surgery surgical microscope,operation microscope,operating microscope), radial and ulnar clubhand,syndactyly,and cerebral palsy.In association with orthopaedic oncology (Hand Surgery surgical microscope, Operation microscope, Operating microscope) service, treatment of benign and malignant tumors of the upper extremity is included in the education of the residents and fellows.

An extensive orthopaedic (Hand Surgery surgical microscope,operation microscope, operating microscope) research laboratory including a hand & microvascular lab is available to the residents and fellows. A modern fresh cadaver laboratory fully equipped with hand  instruments; microscope (Hand Surgery surgical microscope, operation microscope, operating microscope) is available for practice of surgical procedures and anastomisis dissections of the upper extremity. It allows for extensive research opportunities as well as development of micrology skills. A comprehensive orthopaedic (Hand Surgery surgical microscope, Operation microscope,Operating microscope)  library, which includes all major journals, is available 24 hours is located in the orthopaedic offices.
